Yea, it happens to me all the time.
Even on a deeper level, for example in Photoshop when I am looking at
one layer but a totally different layer is active...

The solution is not that difficult - in theory. You need a web cam or
alternatively your mobile phone/pda camera pointing at your face
transmitting your eye position and the head area to a application focus
manager...
