This is a good solution. Alternatively David, in the same widget you
mentioned, remove confusion by placing the select list first and the text
field later with labels something like:
| select from list ^ |
or | write your own |
The advantage it has over Mike's solution is in scenarios a] where users
abandon the form without clicking the drop down, therefore never discovering
that an option exists for entering their own choice and b] when the drop
down options are so many that the user may not scroll all the way down to
the choice.
Also, you can use color coding [same background for both the fields] to help
users distinguish between this data field and the others.
