A few months ago (I seem to recall) there was a discussion about online
forms. Is there a summary of this discussion somewhere?

Our team has just been handed an assignment to re-think the form process for
our site's "mail me information about [1 of 400 topics]" -- for both
prospects and customers. Our current process would discourage all but the
most assiduous user. Can anyone recommend a resource that addresses this
topic?
