Deb, I use QuickBase for this a lot (and I would use it even if I didn't
work for the company that owns it). QuickBase is a web-based flat file
database that is very versatile. You can create up to 3 databases up to 1
MB in size free of charge.

I've seen surveys done with QB two ways. The first way is to code an HTML
page that feeds the data into a QuickBase record (more elegant, but more
work). I've done this a number of times with excellent results.

The second way is to have the questions be the same as the names of the
QuickBase fields, then send people directly to the QuickBase to enter a new
record. Less elegant, but less work, and results in the db are easy to
share because the entire question is visible.

I would be happy to share some examples with anyone who is interested.
