I've got the following roundup of suggested survey tools and services
back from Brett, the guy who is looking into this within our company.

We haven't found anything which fits our needs well. Basically we want an
afforable solution for providing for NZ Education users to run surveys
through our site. Raosoft.com's offering looks good, but would be too
expensive.

We are contemplating doing an in-house development. I've suggested that
there is a need for a quality open-source offering in this area, and it
might be possible to find others to contribute to it's development.
Initially we'd hope for financial involvement from others who want an
affordable solution themselves, and perhaps want to see their required
features included. Development assistance would also be helpful if we can
sort out how to coordinate it. In the longer term an open-source model
would hopefully mean others contributing feature enhancements and
bug-fixes.

It's by no means certain that this is how things will pan out, but if
people are inclined to contribute, I'd certainly like to hear from them.

We didn’t want a Hosted service or ongoing costs or pricing per respondent etc etc.

Note that I could be wrong in some cases where I have stated they are only a hosting service. This was what I gleaned from a brief scan of their sites. I would be pleased to be proved wrong.
