There are many examples of applications to address this
issue, but they mostly miss the point. The key to knowledge
sharing lies in building an empathetic culture, recognition
and reward of aligned behaviors, example and leadership
from top executives and modeling desired behavior by
a core of evangelists.
Reliance on systems, fancy workflow, advanced natural
language search, pattern recognition and content rating
schemes just does not deliver the goods.
If I may venture some advice, concentrate on things cultural
first, let the systems part lag. What is needed is a directory
so people can find people, relationship building and opportunities
to explore the needs and learning desires of cohorts. Then
make contact information easy to enter and attach it to
all key objects. Steer away from the 'knowledge exchanges',
FAQ engines and other tools and look for a system that wrapped
conversation around digital artifacts. Intraspect does this well.

But the real answer does not lie with systems!
If you wish to did deeper here, look at "Common Knowledge"
by Nancy Dixon, she gives many processes for knowledge
transfer and has a neat taxonomy for this domain in her book.
